Carelon Health, CarelonRx to Offer Free Medication Reviews Leading up to National Check Your Meds Day

Carelon Health (formerly CareMore Health) and CarelonRx will offer free medication reviews to members of the public Oct. 16-20, leading up to National Check Your Meds Day.

That week, members of the public are encouraged to call our tollfree number – 1-833-664-0089 – and speak to a Carelon pharmacist who will review patients’ complete list of medications (prescription and over-the-counter medications) free of charge.

During the medication review, pharmacists will help people review their medications, identify any potential medication interactions that they might not be aware of, offer suggestions for discarding expired medications, and identify barriers to care. They will also answer any questions people may have about their medications.

Routinely reviewing your medications is important because many patients have several doctors, and more than 40% use more than one pharmacy. In addition, the FDA receives over 100,000 reports associated with medication errors every year.

According to Carelon Health and CarelonRx pharmacists, the people who may benefit the most from a medication review include those:

  • Taking several medications (five or more) regularly
  • Routinely taking over-the-counter medications, vitamins or supplements
  • Obtaining prescriptions from more than one pharmacy or provider
  • With multiple chronic health conditions such as diabetes or heart failure
  • Recently hospitalized or transitioned to another care setting (such as a skilled nursing facility)

“National Check Your Meds Day is a great reminder to speak to a pharmacist about the complete list of medications you are taking,” said Dr. Khyati Gupta, medical officer at Carelon Health. “A medication review can help people identify potential drug interactions and potential side effects, and even help patients identify cost-savings solutions.”

“During a medication review, we also have the opportunity to help identify barriers to care that may keep patients from staying adherent to their medications, such as lack of transportation, or food insecurity. It’s a moment to spend quality time speaking with patients one-on-one, connect them to community resources if needed, and contribute to better health outcomes,” said Ami Bhatt, Pharm.D., staff vice president of clinical pharmacy services at CarelonRx.
